Sciatica

Pain travelling from the back down into the leg means a nerve is being irritated. It is frightening and it can be severe — but in most people it settles without an operation.

Do not wait for an appointment

Do not wait for morning. Do not wait for an appointment. This is the one page where hours matter.

  • You cannot pass urine, or you are leaking urine or stool without knowing
  • Numbness between your legs, around the back passage, or in the area you sit on
  • Numbness or pain now affecting both legs
  • Weakness in the leg or foot that is getting worse — you are tripping, or dragging your foot
  • Sudden loss of sexual sensation
  • Fever, or you have had cancer in the past

Do this

  • Keep moving, even a little. Short frequent walks beat lying still
  • Find the positions that ease it — for many people lying with the knees bent, or leaning forward on a trolley when walking
  • Take painkillers regularly for the first days rather than waiting for the pain to peak
  • Heat for the back muscles that go into spasm alongside it
  • Start the nerve glides on Sheet 03 once the very worst has passed
  • Keep the red box above on your phone or your fridge, and tell whoever is at home what to watch for

Avoid this

  • Bed rest. More than a day or two makes recovery slower
  • Rushing to an MRI in the first weeks unless there are warning signs
  • Forceful manipulation while the leg pain is severe
  • Ignoring a foot that is becoming weak because the pain is improving
  • Assuming an operation is inevitable. Most people never need one

The questions everybody asks

How long does it take?

Most people improve substantially over six to twelve weeks.

The leg pain usually settles before the numbness does, and a patch of numbness can linger after everything else has recovered. That is normal and it is not a sign of damage.

Do I need an MRI?

Only if we are considering surgery, or if there are warning signs.

An MRI in the first weeks does not change what we do — and it will show disc bulges that are present in large numbers of people with no pain at all. We order it when the result would change the decision. Otherwise it costs you money and often frightens you for no reason.

Will I need surgery?

Roughly one person in ten with sciatica ends up having it.

Surgery relieves leg pain faster in people who are not settling. At one to two years the results of surgery and of waiting are much closer than most people expect. So unless there are warning signs, it is usually reasonable to give it time — and that is a choice you make, not one made for you.

Does an injection help?

It can reduce the pain for some people. It does not change the long-term outcome.

It is worth considering when the pain is severe and not settling, as a way of getting through a bad period. It is not a cure and it does not prevent surgery.
