After your knee replacement

The operation gives you a new joint. How well it straightens and bends depends on the exercises on the sheet, done a little at a time, every day, from the day you go home.

Do not wait for an appointment

These mean come the same day, not at the next appointment.

  • Sudden breathlessness or chest pain — go to the nearest hospital
  • The calf becomes swollen, tight or painful
  • Wound redness, discharge or fever
  • The knee is hot, red and swollen, or you have a fever
  • You cannot put any weight on the leg
  • The foot becomes numb, cold, pale or blue
Sheet 31 · After knee replacementHip & kneeImproves steadily; often 6–12 months to fully settle

Do this

  • Get the knee fully straight first. A knee that does not straighten fully leaves you with a limp
  • Do the sheet a little at a time, several times a day. Short sessions done often loosen the knee better than one long painful one
  • Take your pain tablets about half an hour before the exercises, as prescribed, so that pain does not stop you
  • Put ice on the knee for 15 minutes after exercising, wrapped in a cloth, and keep the leg up when you sit
  • Keep taking the blood-thinning medicine for as long as it was prescribed, even when you are walking well
  • Use the walker or stick until we tell you to stop
  • Get any infection anywhere in the body treated promptly — skin, urine or teeth. Tell any doctor or dentist that you have a knee replacement

Avoid this

  • A pillow under the knee. It feels comfortable, and it is how a new knee stiffens in a bent position
  • Hard massage or oil rubbed into a knee that is swollen or recently injured
  • Hot fomentation or anything applied to the wound until it has fully healed
  • Squatting, sitting cross-legged on the floor or using an Indian toilet until we have told you it is safe
  • Pushing through sharp pain at the wound. Discomfort while the knee stretches is expected; sharp pain is not

The questions everybody asks

Why is the knee still warm and swollen?

That is normal for months.

A replaced knee stays warmer and puffier than the other one for three to six months, and often longer. It is usually worse in the evening and after a long walk. What is not normal is warmth with a fever, a red or leaking wound, or a knee that suddenly becomes much worse — those are in the list above.

Why does straightening matter more than bending?

Because you walk on a straight knee.

If the knee cannot straighten fully, the thigh has to work with every step and you limp. Bending improves steadily with the heel slides over the weeks. A knee that has stiffened bent is much harder to correct later, which is why the towel exercise on the sheet matters so much early on.

When can I stop using the walker?

When we tell you, not when it feels fine.

Most people move from a walker to a stick and then to nothing, but the timing depends on your balance, the strength in your thigh, and what was done at the operation. Walking without support too early is how falls happen. We will tell you at your review.

How long does it take?

Daily activities within about six weeks; the knee keeps improving for up to a year.

Most people are back to everyday activities within about six weeks. After that the knee keeps improving more slowly — less swelling, less stiffness, more confidence on stairs — for up to a year. Keep doing the exercises well after the knee feels good; that is what keeps the thigh strong.

On cost: if we suggest a scan, an injection or an operation, ask us what it costs and what happens if you do nothing. We will answer both. You should never feel pushed into spending money.

Consultation is ₹500 for a first visit and ₹300 for a follow-up, the same at both clinics. For anything else, ask at the desk or on WhatsApp before it is done — we will tell you the exact cost.

Should you come and see us?

A page is a starting point, not a diagnosis. If the pain is not settling, come in and have it looked at properly — bring any X-ray, MRI or CT films you already have.
