Meniscal (cartilage) tear

The meniscus is the shock absorber inside the knee. Tears happen two ways — a sudden twist in a younger knee, or gradual wear in an older one — and the treatment is completely different for each.

Do not wait for an appointment

These are uncommon — but they need to be seen now.

  • The knee is locked and will not straighten fully — this may need surgery soon
  • You cannot bear weight
  • The knee is hot and swollen with a fever
  • Rapid swelling within an hour of a twisting injury

Do this

  • Distinguish which kind you have — sudden twist in a young knee, or gradual onset in an older one
  • If it came on gradually: exercise first. Sheet 37, given three months
  • Strengthen the quadriceps
  • Keep walking

Avoid this

  • Rushing to keyhole surgery for a degenerative tear. Good trials show exercise works as well, and surgery carries risk for no added benefit
  • Assuming a tear on the MRI is the cause of the pain — tears are found in most painless knees over fifty
  • Resting the knee

The questions everybody asks

The MRI shows a tear. Should it be removed?

In an older knee that came on gradually — usually not.

Meniscal tears are present in a large proportion of people over fifty with no knee pain at all. Removing one that is part of the ageing process does not reliably help, and losing meniscus makes arthritis more likely later.

When IS surgery right?

A genuinely locked knee, and repairable tears in younger people after a clear injury.

A knee that truly cannot straighten needs treating. And in a young person a repairable tear should be repaired, not trimmed away.

What if the exercise does not work?

Then we reconsider — but after three months, not after three weeks.

A large proportion of people who begin an exercise programme never end up needing an operation.

Consultation is ₹500 for a first visit and ₹300 for a follow-up, the same at both clinics. For anything else, ask at the desk or on WhatsApp before it is done — we will tell you the exact cost.
