Pain on the side of the hip

Pain on the bony point on the outside of the hip, worst lying on that side at night and going up stairs. It is a tendon problem — and the positions you hold all day matter as much as any exercise.

Do not wait for an appointment

These suggest something else.

  • Pain felt in the GROIN rather than the outer hip
  • Pain after a fall — especially if you are older
  • You cannot bear weight
  • Fever, redness or swelling
  • Numbness or pain travelling below the knee

Do this

  • Sleep with a pillow between your knees, and not on the painful side
  • Stand on both feet evenly. Do not hang on one hip — this squashes the tendon all day
  • Sit with the knees apart, not crossed over the midline
  • Isometric and gentle strengthening — Sheet 34
  • Take the stairs one at a time while it settles

Avoid this

  • Stretching it across your body. The classic figure-four stretch compresses the tendon and makes this worse
  • Lying on the painful side
  • Repeated steroid injections
  • Standing with your weight on one leg — this is the single biggest habit to change

The questions everybody asks

Why does it hurt so much at night?

Because lying on it squashes the tendon against the bone.

The pillow between the knees and not lying on that side often makes a noticeable difference within a week or two — before any exercise has had time to work.

Is it bursitis?

That is the old name; the tendon is usually the real problem.

The name matters because it changes the treatment — tendons need loading, whereas a bursa was treated with rest and injections.

Do I need a scan?

No, in most cases.

The diagnosis is made by pressing on the bony point and by the story of night pain.
