Kneecap pain

Pain at the front of the knee, worse going down stairs, squatting, or after sitting for a long time. It is not arthritis, it is not a torn cartilage, and it responds very well to the right exercises.

Do not wait for an appointment

These suggest something else.

  • The knee locks and will not straighten
  • It gives way completely rather than feeling weak
  • Significant swelling
  • Pain after a direct blow or a twisting injury
  • Night pain, fever, or weight loss
Sheet 32 · Kneecap and hip exercisesHip & kneeUsually improves over 3–6 months with exercise

Do this

  • Strengthen the hip, not just the knee. Most kneecap pain is a hip control problem — Sheet 32
  • Practise the single-leg squat in front of a mirror, keeping the knee over the second toe
  • Reduce deep squatting and stairs temporarily, not permanently
  • Build back up gradually — this responds to loading, not rest

Avoid this

  • Complete rest. The muscles waste fast and the pain returns worse
  • A knee brace as a substitute for strengthening
  • Keyhole surgery. There is no structural problem to fix here
  • Expecting change in two weeks — give it three months

The questions everybody asks

Do I need a scan?

No.

This is a diagnosis made by examination. An MRI will find incidental findings and confuse matters.

Why are you giving me hip exercises for a knee problem?

Because the knee follows the hip.

When the hip muscles are weak, the knee collapses inwards on every step and the kneecap is loaded badly. Strengthening the hip fixes the cause; strengthening only the knee treats the symptom.

Will this turn into arthritis?

There is no good evidence that it does.

This is a loading and control problem, not the beginning of joint destruction.

On cost: if we suggest a scan, an injection or an operation, ask us what it costs and what happens if you do nothing. We will answer both. You should never feel pushed into spending money.

Consultation is ₹500 for a first visit and ₹300 for a follow-up, the same at both clinics. For anything else, ask at the desk or on WhatsApp before it is done — we will tell you the exact cost.

Should you come and see us?

A page is a starting point, not a diagnosis. If the pain is not settling, come in and have it looked at properly — bring any X-ray, MRI or CT films you already have.
