Knee arthritis

Arthritis means the cushioning in the knee has thinned. It does not mean your knee is crumbling, and it does not mean you are heading for a wheelchair.

Do not wait for an appointment

These suggest something other than arthritis.

  • The knee is hot, very swollen and you have a fever — this can be infection and it is urgent
  • You suddenly cannot put any weight on the leg
  • The knee has locked and will not straighten
  • It swelled up rapidly within an hour of an injury
  • Severe pain at night that does not ease however you lie
  • You are losing weight without trying, or you have had cancer in the past

Do this

  • Walk. Movement feeds the joint — this is treatment, not a risk
  • Strengthen the thigh muscle. A strong quadriceps takes load off the joint and reduces pain more reliably than any tablet
  • Heat before activity; cold after, if it swells
  • Simple painkillers before the activity you know will hurt, rather than after
  • A walking stick in the hand OPPOSITE the painful knee — this is the commonest thing people get the wrong way round
  • If you are overweight, even a few kilograms off makes a measurable difference at the knee
  • A chair or commode at home if squatting and floor-sitting have become painful

Avoid this

  • Resting it. Rest is the single worst thing for an arthritic knee
  • Keyhole surgery or a "washout". Good trials have shown it does not help arthritis — we will not offer it and you should not pay for it
  • Expensive oil or gel injections into the joint. The evidence does not support them
  • Supplements and powders sold for joints
  • A knee brace bought off a shop shelf and worn all day
  • Assuming you must stop walking. That makes everything worse

The questions everybody asks

Do I need a scan?

An X-ray sometimes. An MRI almost never.

A standing X-ray tells us what we need. An MRI in an arthritic knee finds meniscal tears in most people over fifty — including those with no pain — and those tears are part of the arthritis, not a separate problem to be operated on.

Will exercise wear the knee out faster?

No. The opposite.

This is the most common and most costly misunderstanding. Cartilage is fed by movement. An unused knee gets stiffer, the muscles waste, and the pain gets worse. Walking is treatment.

Do I need a knee replacement?

Only when the knee stops you living your life — not when the X-ray looks bad.

The X-ray does not decide. You do. When pain wakes you at night, when you cannot walk to the shop, when you have stopped doing things you care about and exercise and painkillers are no longer enough — that is the time. There is no benefit in having it earlier, and a replacement has a finite lifespan, so a younger knee may need revising later.

Will I be able to squat and sit on the floor afterwards?

Honestly — most people cannot squat deeply after a knee replacement, and kneeling stays uncomfortable.

We say this before the operation, not after. If daily life, meals or prayer happen at floor level, plan for a chair and a commode, and tell us — it is part of deciding whether and when to operate.
