Heel pain

Pain under the heel, worst on the first few steps in the morning and after sitting. It is not caused by the spur, and it does settle — but slowly, and the stretching is what does it.

Do not wait for an appointment

These suggest something other than ordinary heel pain.

  • The pain started after a fall from a height or a hard landing
  • The heel is red, hot and swollen, or you have a fever
  • Numbness, burning or pins and needles in the sole of the foot
  • You are diabetic and there is any break in the skin
  • Both heels are painful in a young adult who also has morning back stiffness
  • You cannot put weight on the heel at all

Do this

  • Stretch before your first step. Sitting on the edge of the bed, pull your toes back towards you and hold — this is the single most effective thing you can do, and it must happen before you stand up
  • Calf stretches with the knee straight and with the knee bent — they stretch different muscles and you need both
  • A cushioned heel and a stiff-soled shoe
  • Keep a pair of chappals by the bed and wear them indoors. Hard floors barefoot are what keeps this going
  • Roll the sole over a cold bottle for a few minutes in the evening
  • Reduce long periods of standing where you can

Avoid this

  • Walking barefoot at home. This is the commonest reason heel pain does not settle
  • Paying for custom-made insoles. Trials show they work no better than a good ready-made one from a shop
  • Steroid injections early — they carry a real risk of rupturing the fascia or thinning the heel pad
  • Surgery to remove the spur. The spur is not the problem
  • Stopping the stretches after two weeks because nothing has changed yet

The questions everybody asks

Is it the spur on my X-ray?

No.

Heel spurs are found in very large numbers of people with no heel pain whatsoever, and plenty of painful heels have no spur at all. The spur is a bystander. Removing it does not treat this.

Do I need a scan?

No.

This is diagnosed by where it hurts and when. An X-ray is only useful if we suspect something else — a stress fracture, for instance, after a fall.

Should I buy custom insoles?

No. A cushioned, supportive ready-made insole does the same job.

Custom orthoses are frequently sold for this and the evidence does not justify the cost. Spend the money on a decent pair of shoes and a pair of indoor chappals instead.

How long before it goes?

Most people are substantially better within a few months, but it can take a year to fully settle.

It improves throughout — it does not stay bad and then suddenly stop. The people who recover fastest are the ones who do the morning stretch every single day and stop walking barefoot indoors.
