Achilles tendon pain

Pain and stiffness in the tendon at the back of the ankle, worst first thing in the morning. Where exactly it hurts matters — because the wrong exercise makes one type substantially worse.

Do not wait for an appointment

A ruptured tendon needs assessing promptly.

  • You felt a sudden snap or a blow to the back of the ankle, often with a pop
  • You cannot push off or stand on tiptoe on that leg
  • There is a gap you can feel in the tendon
  • Redness, heat and fever
  • You have recently taken a fluoroquinolone antibiotic and have new tendon pain

Do this

  • Find out which type you have. Pain in the middle of the tendon and pain right at the heel bone are treated differently — ask us to show you
  • Loading exercises — and check Sheet 41 for which version applies to you
  • A small heel raise in both shoes while it settles
  • Reduce hill and stair work temporarily
  • Be patient. Tendons respond to load, slowly

Avoid this

  • Steroid injection into the tendon. It risks rupture and we do not do it
  • If the pain is right at the heel bone: do NOT drop the heel below a step. That compresses the tendon against the bone and makes it worse — this is the commonest reason people say the exercises made them worse
  • Complete rest
  • Stretching hard into pain

The questions everybody asks

Why does it matter where exactly it hurts?

Because the standard exercise helps one type and harms the other.

Mid-portion tendon pain improves with heel drops off a step. Pain right at the heel bone is made worse by dropping below level — those patients need the floor-level version instead. We will show you which you are.

Could it rupture?

A painful tendon is not on the verge of snapping.

Ruptures usually happen suddenly in people with no previous pain. But a sudden snap with inability to tiptoe needs assessing the same day.

Do I need a scan?

Usually not.

Examination makes the diagnosis. Ultrasound is occasionally useful if the picture is unclear.

Consultation is ₹500 for a first visit and ₹300 for a follow-up, the same at both clinics. For anything else, ask at the desk or on WhatsApp before it is done — we will tell you the exact cost.
