Hammer, claw and mallet toe

A toe that has curled and now rubs inside the shoe. The toe is the part that changed, but the shoe is usually what makes it hurt, and that is the part you can do something about.

Do not wait for an appointment

The skin is what matters here, particularly if you are diabetic.

  • The skin over the toe breaks, blisters or discharges
  • The toe becomes red, hot and swollen
  • You have diabetes and the skin over the toe is broken or discoloured
  • The toe becomes cold, pale or blue
  • The deformity appeared suddenly rather than over months
Sheet 44 · Foot and toe exercisesFoot & ankleFootwear change helps most people

Do this

  • Change the shoe before anything else. A deep, wide toe box with a soft upper does more than any exercise here
  • Measure your feet again. Most people are wearing a shoe half a size too small, and feet spread with age
  • Use a silicone toe sleeve or a gel cap over the rubbing point
  • Do the toe exercises on the sheet while the toe still bends. A flexible toe can be helped; a stiff one usually cannot
  • Have the corn pared properly rather than cutting it yourself, particularly if you are diabetic

Avoid this

  • Corn caps and medicated plasters with acid in them. They burn healthy skin as well, and in a diabetic foot that is how an ulcer starts
  • Cutting a corn or callus yourself with a blade
  • Narrow or pointed shoes, and heels, while it is sore
  • Strapping the toe tightly to the one beside it — that can rub a new sore between them
  • Assuming an operation will let you wear the same shoes afterwards. It usually will not

The questions everybody asks

Why has my toe curled?

The balance of pull across the toe has shifted.

Small muscles inside the foot hold the toe flat while longer tendons bend it. When the small ones weaken or the toe is cramped in a shoe for years, the long tendons win and the toe buckles. Tight shoes, a bunion pushing the toe over, and diabetes affecting the small muscles are the usual reasons.

Will exercises straighten it?

They can help early, not late.

While the toe still straightens when you push it, exercises and a wider shoe can keep it flexible and comfortable. Once it has stiffened they will not restore the shape — but they are still worth doing for the toes either side.

Is the corn the problem or the toe?

The toe, and the shoe.

The corn is the skin protecting itself where the bent toe rubs. Paring it gives relief for a few weeks, but it forms again unless the rubbing stops. That is why footwear comes first.

Should I have it operated on?

Only if the shoe change fails.

Surgery straightens the toe and is usually a small procedure, but the toe is often stiffer afterwards and sometimes slightly shorter. It is a good answer for a painful toe that will not settle, and a poor answer for a toe you simply dislike the look of.

On cost: if we suggest a scan, an injection or an operation, ask us what it costs and what happens if you do nothing. We will answer both. You should never feel pushed into spending money.

Consultation is ₹500 for a first visit and ₹300 for a follow-up, the same at both clinics. For anything else, ask at the desk or on WhatsApp before it is done — we will tell you the exact cost.

Should you come and see us?

A page is a starting point, not a diagnosis. If the pain is not settling, come in and have it looked at properly — bring any X-ray, MRI or CT films you already have.
