Ankle sprain

A rolled ankle stretches the ligaments on the outside. Most heal well — but the ankles that go on to sprain again and again are the ones that never did the balance training.

Do not wait for an appointment

These are the rules we use to decide.

  • You cannot take four steps on it, either straight after the injury or now
  • Tenderness over the bone at the back of either ankle bone
  • Tenderness over the bone on the outer side of the mid-foot
  • The ankle looks deformed
  • The foot is numb, cold or pale

Do this

  • Ice, elevation and a compression bandage for the first days
  • Start moving it early. Ankles stiffen quickly
  • Do the balance training. Standing on one leg, progressing to eyes closed — Sheet 42. This is the part that prevents the next sprain, and it is the part everyone skips
  • Return to sport gradually, with the balance work continuing

Avoid this

  • Prolonged immobilisation in a boot for a simple sprain
  • Returning to sport as soon as the pain settles but before balance has returned
  • Ignoring an ankle that keeps giving way — that is treatable

The questions everybody asks

Do I need an X-ray?

Only if one of the rules in the red box applies.

There are validated rules for exactly this, and they safely avoid a great many unnecessary X-rays.

Why balance exercises?

Because the ligament injury dulls the ankle's position sense.

That is what causes repeat sprains. Restoring it measurably reduces the chance of it happening again — and it takes a few minutes a day.

It keeps giving way. Is that normal?

No, and it is treatable.

Persistent instability after a sprain responds to a proper rehabilitation programme, and to surgery in a minority.
